Cosan Group uses trends in home patient monitoring-Home Care Daily News

The pandemic is pushing more care into the home and forcing patients at home to become better at using technology. For Cosan Group, headquartered in Moorestown, New Jersey, this is a successful combination. This 6-year-old company provides remote patient monitoring, chronic disease care management, and behavioral health integration technology for 200 doctors’ clinics and 700 suppliers in the U.S.
The Cosan Group acts as a backup force for clinicians who provide care at home, and works with patients using this technology to help them receive care.
“If they think the patient needs laboratory work or chest X-rays, they will safely send it to our coordinator,” Desiree Martin, Cosan Group’s Director of Clinical Services, told McKnight’s Home Care Daily. “The coordinator arranges laboratory work or schedules appointments. Whatever the patient needs, our coordinator will do it for them remotely.”
According to data from Grand View Research, the remote patient monitoring industry is valued at US$956 million and is expected to grow at a compound annual growth rate of nearly 20% by 2028. Chronic diseases account for approximately 90% of U.S. health care expenditures. Analysts say that remote monitoring can greatly reduce the rate of emergency department visits and hospitalizations for patients with chronic diseases, including heart disease and kidney failure.
Martin said that primary care doctors, cardiologists and lung disease specialists make up the majority of Cosan Group’s business, but the company also works closely with many home health agencies. The company provides tablets or apps for patients, which they can download on their devices. This technology enables Cosan Group to monitor patients. It also allows patients to conduct remote medical visits and track their appointments.
“If they encounter a problem and can’t get the device to work, they can contact us and we will guide them to solve the problem,” Martin said. “We also use home health workers as our voice in the room to guide patients through because they are at home with them.”
Martin said that an artificial intelligence tool launched by the company at the end of last summer is quickly becoming one of Cosan Group’s most successful products. “Eleanor” is a virtual assistant who calls patients every week, has 45-minute conversations, and sends out alerts about possible dangers.
“We have a patient who mentioned suicide many times on the phone,” Martin explained. “She finally had a 20-minute conversation with Eleanor. Eleanor tagged her. That was after the practice, so we were able to get in touch with the doctor. She was just in the hospital and he was able to call her and immediately demote .”
